Request for HISD Service Record

This online form may be used by former HISD employees to request an official record of their service (i.e., employment) with the district. If you have any problems submitting the online form, please enter all of your information in the form, print it and fax to
713 556-7362.

NOTE: It is HISD policy to prepare one original service record. If HISD has provided you with an original service record in the past, please contact the school district where you were most recently employed to obtain the original.

If the service record is required for additional teacher certification purposes you should e-mail kgrant3@houstonisd.org for last names beginning A–K and gmaurici@houstonisd.org for last names beginning L–Z.
